Running with Knee Arthritis: Does Cold, Damp Weather Make It Worse?

Current best evidence shows recreational running is not associated with an increased risk of knee osteoarthritis, and may even be protective compared with a sedentary lifestyle. High-volume competitive running (very long distances over many years) shows a higher association with joint issues than moderate recreational running. What the evidence actually shows about running itself

A meta-analysis of 17 studies covering more than 14,000 patients (roughly 7,000 runners and 7,000 non-runners) found no evidence that regularly running increases the risk of developing knee osteoarthritis or worsens existing knee osteoarthritis — and found a significantly higher prevalence of knee pain in the non-runner group than in the runner group.

A separate systematic review and meta-analysis in the Journal of Orthopaedic & Sports Physical Therapy found that recreational runners had a lower occurrence of osteoarthritis compared with both competitive runners and sedentary controls; running exposure of less than 15 years was associated with lower odds of hip or knee osteoarthritis compared to non-runners. Only 3.5% of recreational runners in one underlying study had hip or knee arthritis, versus 10.2% of sedentary non-runners.

Where the risk actually goes up

The same body of research is consistent that competitive, high-volume running (commonly defined in these studies as more than roughly 57 miles/92 km per week) does carry a higher association with joint issues than recreational-level running. Being overweight remains a far more strongly validated risk factor for knee osteoarthritis than running itself — researchers note that being just 10 pounds overweight increases the force on the knee by 30–60 pounds with each step.

Why outdoor conditions still matter, even though running itself is fine

Separately from the running-specific research above, the broader osteoarthritis literature shows that cold and humidity are each linked to more joint pain, and that the combination of cold-and-damp is linked to more pain than either alone. Outdoor runners in cold, wet climates are directly exposed to exactly this combination for the duration of every run. There is no direct trial testing thermal leggings specifically during running, but the underlying rationale — keeping the joint warmer and drier during cold, damp activity — follows directly from that broader weather-and-pain research, not from any claim about compression or performance.
